The piriformis may be a secondary issue. This may indicate a VERY irritable muscle, in which case other treatment options such as injection therapy might be considered.

Actually not completely correct. In cases where the muscle is VERY irritable, you're right, this may be too aggressive. But for the vast majority of cases, this exercise works like a charm. Hi doc, can you please help me with the below problem. I can't stand for more than a few minutes, because I get pain in my lower legs and feet. 2-4 times a day is usually what I recommend. More won't be a problem. Hold each stretch for at least a minute.
going to try this in a minute. 2 months ago I went to the ER because of the pain from the nerve and the numbness in my leg. I've since gotten back to excersize and basketball but still have occasional numbness and it causes my foot to walk flat footed because I have limited control of it. Would be awesome if this stretch and excersize help.
If you're having trouble controlling your foot, you may have a more serious problem than piriformis syndrome. Typically, the symptoms you're describing are the result of some form of nerve compression. I doubt very seriously this exercise will help. My advice: have the weakness checked out by a specialist.

Throughout the day alternate walking slowly & gently around the house (using a cane or walking aid if you need to) and gel pack because lying down all day will also weaken your back & make it worse. I just want to say thank.you so much. I'm a paramedic and got a sciatic attack while lifting a patient today. I fell to the floor in pain and couldn't walk. This stretch, along with some similar stretches I've been doing that seem to target the same area (e.g. pigeon stretch), are EXCRUCIATING, and haven't gotten any better with continued stretching. Anybody know what's wrong?

It works- I do this every night in bed not on a floor mat. My Psyhiatrist/Trainer both suggested to do this exercise at 3-4 x's a week. This will stop the Piriformis muscle from pressing on the Sciatica Nerve. Have not had any pain since 8/2012.

Comments about this video: My problem begins at the Piriformis (muscle spasms in area on day 2). Typically prescribed stretching exacerbated the condition. What - it gets worse until it gets better (until muscle stretches)? There is often some underlying cause for the muscle to be tight and if it's not addressed, the muscle will remain tight no matter how much stretching you do. Common issues are jamming/dysfunction of the sacroiliac and/or hip joints and disc bulges that are bad enough to irritate the nerves to the piriformis but not enough to cause sciatica. Many family doctors are ill-prepared to diagnose and treat back pain issues. While he may or may not be correct that you have arthritis, in most cases there are treatments that will work much better than pain medication. My suggestion would be to request a referral (or just go on your own) to a chiropractor or physical medicine doctor to be evaluated and to get a more specific diagnosis. Once you have a better idea of what the problem is, you have a better chance of getting effective treatment.
Swimming is usually good, although it can temporarily increase symptoms when there is severe inflammation or muscle spasm present. To reduce the chance of increasing symptoms, start at a slow pace and just do a few minutes at a time and gradually increase the time if all goes well.
2 DrGeorge I have been having an issue 4 a longtym & went 2 doctors but none could knw whats the matter. i have been sucking in my stomach every 2 minutes I feel there is a muscle that is held in my stomach & I have to breath in 2 press on it as if there is something tangled in my lower stomach. Whats wrong with me? & whats the solution? Based on symptoms alone, I can't say what's wrong or what to do about it. You really need to see a doctor who is going to do the appropriate tests to try to find out what's wrong. Most workouts can be tolerated just fine once the disc issues are under control, but high-impact activities like running and certain exercises like that place the spine in flexion can aggravate disc issues and may need to be avoided. Bear in mind that disc herniations don't really get "fixed" per se, but they can be managed (such as with the self-treatment and prevention methods in my other videos and on my website) so that they don't cause significant symptoms. Dr I fell of the stairs in a sitting postion it didn't hurt that much until 2 days later I went do some sport and then I started having pain in my lower back and my legs I thought it was a herniated disc at first but since the pain is getting better I don't know what it is now and i'm not sure if it is sciatica. what do you think? It could be a herniated disc with sciatica, but it could easily be due to just inflammation and muscle reactions and possibly some joint dysfunction from the trauma of the fall. In either case you might want to try the methods in the free ebook on my website to try to speed up the recovery and if none of those methods help, it would probably be a good idea to get checked by a chiropractor to get the joints checked out to make sure they are aligned and moving properly. When I get up from a chair, or move in bed is when I get a lot of pain. When getting of a bed or couch the only way I can is by rolling of the side of the furniture. All other times I feel a dull ache. When I do begin to walk the pain is severe for the first 15 steps or so then subsides to an ache. Does this sound like Sciatica? Tried the two chair tests. Test 1 - Stretching out leg while sitting): Left area of buttock and top of leg very painful when stretched out. Right leg fine. Test 2) Crossing Legs While Sitting: No real pain from either side. Sciatica can sometimes be limited to the buttock area, but the description of your symptoms sounds more like a sacroiliac joint issue. The self-treatments on my website are worth a try, but if you don't get much help from those, I'd suggest seeing a chiropractor. I slid and felt on my sacrum. I been having tingling and nerve (electric) spasms on both feet. Neuro thinks is neuralgia from the accident. Can i have sciatica without having radiating/traveling spasms? i dont feel the nerve pain running thru my leg. Instead i get sporadic nerve spasms on foot.It's probably not so much sciatica as inflammation in the pelvis / low back irritating one or more of the individual nerve roots in the lower spine (the sciatic nerve is made up of multiple roots). If this was a fairly recent injury (within the past month or so), chances are things will get better with time. You didn't mention having an MRI, but if the problem is not improving much after a couple of weeks, I would suggest talking to your neurologist about doing one to evaluate further. Can ITB syndrome be "connected" to this sciatic/priformis?? not emanating from the spine but from knee /acl issues thanks decades of martial arts :). You could have ITB with sciatica/piriformis syndrome, but they are different conditions. A knee issue could cause alteration of gait that could lead to increased mechanical stress on the hip/sacroiliac/lumbar spine and in turn initiate disc or piriformis related symptoms.I think I got sciatica after taking a bump on my buttocks while playing basketball. If you fell on your buttocks, it could be either disc or piriformis related sciatica. You forgot to mention a couple of things Doctor. Firstly studies have shown that people walking around with the disc bulges and degeneration don't all have pain. People also have sciatica with a good healthy spine. Also people who have their disc surgically fixed don't always get lasting relief from the pain. Gravity + muscle imbalance = unbalanced pressure on the spine. It happens before any of your stuff & is a constant in sciatic pain sufferers. backpain.com.au. All of what you have said is true, and it is definitely important to address underlying mechanical problems that precipitate the more severe issues that ultimately produce symptoms in most cases. That being said, most people don't recognize that they have a problem until they develop symptoms and they already have these more severe issues.In some cases, treating the original imbalances will be quite effective, but in terms of getting rid of symptoms, it is usually more efficient to address the disc bulging and other severe issues first, and then deal with corrective measures to prevent future problems. My videos and website are intended to give a basic understanding of the immediate problem and give people methods they can use to alleviate and then prevent their own symptoms without having to undergo expensive treatment. I respect what you are saying Doctor but in my vast experience the disc issues have actually resolved by correcting the mechanical problems. When the pressure comes off the disc it recovers on its own, often within days. I am sure you would agree that avoiding surgery and invasive treatments to the spine is a priority but unfortunately too many doctors tell people surgery is their only option when they see a disc bulge or herniation.
I definitely agree that anything that helps people avoid surgery is a good thing and I think your method is great. We're jut taking a slightly different approach to accomplish basically the same thing (most of the cases I see also resolve symptomatically within a few days). While I am also a practicing clinician, my focus with regards to my internet information is to provide simple but effective self-treatment methods.
While your technique looks to be excellent, and I see that you do provide some self-treatment tips, it appears that your focus is on practitioner training. When your audience is practitioners, you need to provide the detailed explanations you do. When your audience like mine is patients, I think a much simpler approach works better. For them, my goal is to explain what they may have already heard from their doctor and then provide them with the ability to take charge of their own care. I have been in constant pain for over 4 years. Extreme left buttock pain and burning lower back pain that is unbearable. No doctor will give me any answers. I have had a mdi and been told that I have a bulging tear inbetween at L3. You didn't say what treatment you've had, but with the bulging disc, you might benefit from the self treatment methods in the ebook on my website (link in the video description). Depending on the severity of the disc problem, you might require more aggressive treatment, but it at least will give you a place to start.
Ok I think I may have Sciatica... I am going to get checked out today. How do drs determine if you have sciatica ? I've been having pains for a month now and it's getting worse.
